TELADOC DIABETES MANAGEMENT:

VISION PLAN:

LIFE INSURANCE:
MTS employees are eligible for $25,000 of basic life insurance at no cost.
DISABILITY INSURANCE:
Short-term and long-term disability insurance are available to employees at group rates.
401(K) PLAN:
After new employees have completed 6 months of service they are eligible to enter MTS's 401(K) Plan. Employees can invest in any combination of 18 mutual funds, and MTS match to the employee contribution is 25%, up to 20% gross pay. Employees are 100% vested in the 401(K) Plan upon entry.
FLEXIBLE SPENDING ACCOUNTS:
After completing one year of service, employees are eligible to participate in the healthcare FSA and dependent care FSA programs.
After completing six months of service, employees are eligible to participate in the MTS 401(K) Plan.

Bereavement Pay/Funeral Leave:
MTS employees shall be granted time off with pay of bereavement and to attend a funeral as follows:

3 paid days: Employee's spouse, child or parent (includes "step" relationships of the same sort)

1 paid day: Employee's brother, sister, brother-in-law, sister-in-law or spouse's parent (included "step" relationships of the same type)
